I do agree with that to a certain degree, however, If it's a regular client and he's booked me for an overnight in London, I've usually had a 4 hour journey to get to him (cab/metro in Newcastle to station, 2 and half hour train journey to London and 30 mins underground at the other end) so I will always have a shower when I arrive at his hotel, maybe not straight away, especially if we're having a glass of wine beforehand but it'll tend to be in his time. If he's in no hurry in the morning, I will tag 30 mins onto the time for taking time out, if he's on constrained times then, its tough, I have to shower after a 4 hour journey otherwise he won't have me at my best and sometimes it has to be in his time. This would never happen on a 2 hour appointment though, the clock would always start once I am ready.

I agree with you in that scenario. If it is a 12 hour appointment and the escort has travelled a long distance of course he is entitled to time to freshen up. But for a one hour appointment especially if I am traveling to him , I would expect him to be fresh and ready unless having a shower together was part of the deal.
